Without insurance, a hospital in Bali will not treat you — or not beyond the first emergency measures — until it has a guarantee that someone will pay. France's foreign ministry puts it bluntly in its Indonesia travel advice (updated 4 September 2026): "Indonesian hospitals provide no care until they have a coverage agreement from an insurer or, failing that, a credit card number to charge." In practice, the broker Insurance Indonesia puts the deposit demanded from an uninsured patient at USD 500 to 5,000 on admission to a private hospital. After that, everything hinges on which hospital you land in: a night in a standard room costs IDR 3 to 5 million at BIMC (USD 188 to 313), IDR 1.5 to 3 million at Siloam, IDR 800,000 to 1.5 million at Kasih Ibu and IDR 400,000 to 800,000 at Sanglah, the public hospital in Denpasar (Bali Zero comparison, February 2026). An emergency-room visit runs from IDR 100,000 in the public system to IDR 10 million at BIMC; a night in intensive care at an international private hospital, USD 1,000 to 2,500. The textbook scenario documented for 2026 — scooter crash, broken leg, surgery and five nights at BIMC or Siloam — easily tops USD 20,000 to 30,000. An evacuation to Singapore adds USD 20,000 to 40,000 on a commercial flight with a medical escort, USD 50,000 to 100,000 by dedicated air ambulance. The only genuinely cheap rate card belongs to the public RSUP Prof. Ngoerah hospital (formerly Sanglah): paying out of pocket, a VIP room and intensive care both come to IDR 550,000 a day (about USD 34) — room only, before doctors' fees, drugs and tests.
Why "no insurance" changes everything at the front door
The rule is simple: in Bali you pay before you are treated, or you prove that someone else will. The French foreign ministry's advice (France Diplomatie) adds three sentences that set the scene: "to cope with hospital and healthcare costs, which can be very high abroad, it is imperative to hold an assistance contract or insurance"; "private hospitals are to be preferred over public hospitals"; and "in the event of a serious health problem, an evacuation to Singapore or France will have to be considered". Swap France for your own country and the logic holds for any foreign passport.
On the ground, a guide published on 20 April 2026 by the tour operator Your Happiness Tours describes a "financial guarantee" demanded from uninsured patients once they have been stabilised — a sizeable cash deposit, a bank-card pre-authorisation or a guarantee from an embassy — before any non-urgent treatment continues. How much? The broker Insurance Indonesia, in its guide to Bali's hospitals updated in January 2026, puts it at USD 500 to 5,000 depending on the estimated cost of care, as a cash deposit or a bank-card guarantee to be provided before treatment. The source sells insurance and has every reason to lay it on thick, but it is the only one to put a number on it, and its range squares with what France Diplomatie reports: without a policy, you need a card that can take a hit of up to USD 5,000 in one go.
How much does a consultation cost without insurance?
This is the least painful line item, and already the one where the public-private gap jumps out. The Bali Zero comparison (updated 17 February 2026) gives the ranges by hospital; the USD conversions are the source's own, at roughly IDR 16,000 to the dollar.
| Hospital | GP consultation | In USD |
|---|---|---|
| BIMC (Kuta, Nusa Dua) | IDR 800,000 – 1,200,000 | 50 – 75 |
| Siloam (Kuta) | IDR 400,000 – 600,000 | 25 – 38 |
| BaliMed | IDR 250,000 – 500,000 | 16 – 31 |
| Kasih Ibu (Denpasar, Tabanan) | IDR 250,000 – 400,000 | 16 – 25 |
| RSUP Sanglah / Prof. Ngoerah (public) | IDR 100,000 – 200,000 | 6.25 – 12.50 |
The public hospital's official rate card goes lower still: paying out of pocket, IDR 80,000 for a GP consultation and IDR 175,000 for a specialist (details further down).
French-language sources quote noticeably lower figures: the assistance company Europ Assistance lists IDR 200,000 to 500,000 (EUR 12 to 30) for a private GP and IDR 500,000 to 1,000,000 (EUR 30 to 60) for a specialist; the French expat site vivreabali.fr (June 2026) gives IDR 300,000 to 800,000 (EUR 18 to 48) and IDR 500,000 to 1,500,000 (EUR 30 to 90), and EUR 50 to 80 on its insurance-comparison page. The explanation: a neighbourhood clinic and BIMC are two different pricing universes. The English-language guides confirm the top of the scale: GP USD 20 to 100 (Insurance Indonesia) or USD 30 to 70 (Nomedic), specialist USD 40 to 150 or USD 50 to 120, home visit USD 35 to 80. Imaging: X-ray USD 25 to 60 and MRI USD 200 to 450 (Nomedic), "imaging/X-ray" USD 50 to 250 (Insurance Indonesia).

What does an emergency-room visit cost?
Here too, the name over the door weighs as much as the severity of the case. Bali Zero prices an emergency-room visit at IDR 2 to 5 million at BIMC (USD 125 to 313), IDR 500,000 to 2 million at Siloam (USD 31 to 125), IDR 300,000 to 1 million at Kasih Ibu (USD 19 to 63) and IDR 100,000 to 500,000 at Sanglah (USD 6.25 to 31). The insurer Feather (guide updated 17 June 2026) goes up to IDR 3 to 10 million at BIMC: count on IDR 2 to 10 million depending on what is done to you. By level of care:
- registration and initial consultation: USD 50 to 100 (April 2026 guide);
- "basic" visit: USD 100 to 250 (Insurance Indonesia); emergency assessment: USD 100 to 300 (Nomedic);
- consultation plus basic treatment: USD 150 to 300 (April 2026 guide);
- IV drip and medication: USD 200 to 500 (Insurance Indonesia);
- major trauma: USD 3,000 to 50,000 and up (Insurance Indonesia).
A night in hospital: the rate card, hospital by hospital
This is where the sources disagree the most: "a night in hospital" means nothing until you know which hospital. The only comparison that names names is Bali Zero's.
| Hospital | Night in a standard room | In USD |
|---|---|---|
| BIMC | IDR 3,000,000 – 5,000,000 | 188 – 313 |
| Siloam | IDR 1,500,000 – 3,000,000 | 94 – 188 |
| Kasih Ibu | IDR 800,000 – 1,500,000 | 50 – 94 |
| RSUP Sanglah / Prof. Ngoerah (public) | IDR 400,000 – 800,000 | 25 – 50 |
The other sources' all-comers ranges make sense once you read them by tier of hospital: standard room USD 100 to 300 and private room at BIMC USD 300 to 800 (Insurance Indonesia); standard night USD 300 to 600 (April 2026 guide); IDR 3 to 10 million, or EUR 180 to 600 (Europ Assistance); EUR 50 to 120 standard and EUR 120 to 300 VIP (vivreabali). To budget, start from the per-hospital rate card, then add doctors' fees, drugs and tests, none of which the price of the "night" includes.
Six private hospitals are listed by vivreabali: BIMC (Kuta and Nusa Dua), Siloam (Kuta), Kasih Ibu (Denpasar and Tabanan), Bali Royal (Denpasar), Surya Husadha (Denpasar and Nusa Dua) and Prima Medika (Denpasar). On accreditation, Insurance Indonesia flags Siloam as JCI-accredited and BIMC Nusa Dua as ACHS(I)-accredited, the Australian standard — and they are also the two most expensive hospitals in the table.
Surgery and intensive care: where the bill changes scale
The orders of magnitude, procedure by procedure:
- minor surgery at BIMC: IDR 5 to 15 million (USD 313 to 940) according to Bali Zero; minor surgery USD 500 to 2,000 according to Insurance Indonesia;
- major surgery: USD 3,000 to 10,000 and up (Insurance Indonesia);
- simple arm fracture with surgery: USD 3,000 to 6,000; heavy orthopaedics (hip, femur): USD 8,000 to 20,000 (April 2026 guide);
- appendicitis: EUR 1,500 to 3,500; caesarean: EUR 2,000 to 5,000 (vivreabali, June 2026);
- hospital stay for dengue: USD 2,000 to 5,000; hyperbaric chamber after a diving accident: USD 5,000 to 15,000 (Feather, June 2026).
Intensive care is the real cliff edge: USD 1,000 to 2,500 per night in ICU according to Insurance Indonesia, USD 1,500 to 2,500 at an international private hospital according to the April 2026 guide. Feather totals up the worst case, costed on a private hospital in Jakarta: a week in intensive care followed by an evacuation to Singapore can easily reach USD 100,000.
The textbook case: scooter crash, broken leg, five nights
The April 2026 guide prices the scenario every expat has heard at least once: scooter crash, broken leg, surgery, five nights at BIMC or Siloam. Total bill: USD 20,000 to 30,000, easily exceeded according to the guide. The rate cards above let you rebuild it: emergency room and imaging, heavy orthopaedics at USD 8,000 to 20,000, five nights in a room at USD 188 to 313 each, doctors' fees, drugs, consumables — not forgetting the financial guarantee demanded as soon as you are stabilised.

Denpasar's public hospital: the official RSUP Prof. Ngoerah rate card
RSUP Prof. Ngoerah (Rumah Sakit Umum Pusat, the former Sanglah) is Bali's central public hospital, run under Indonesia's Ministry of Health. It publishes its rates on its website (reference "SK Tarif" 2024, Surat Keputusan Tarif Layanan) in two columns: "regular", the track the hospital itself associates with BPJS-insured patients, and "non-regular", the pay-direct track — the most likely case for a foreigner without cover.
| Item (per day or per procedure) | Regular (BPJS) | Non-regular (direct payment) |
|---|---|---|
| Outpatient registration | IDR 10,000 | IDR 10,000 |
| GP / specialist consultation | IDR 40,000 / 90,000 | IDR 80,000 / 175,000 |
| Emergency (IGD): registration / GP / specialist | IDR 25,000 / 50,000 / 90,000 | same |
| Room class III / II / I | IDR 210,000 / 280,000 / 350,000 | not shown |
| VIP / VVIP room | not shown | IDR 550,000 / 1,500,000 |
| Intensive care unit / HCU (intermediate care) | IDR 550,000 / 350,000 | same |
| NICU (neonatal) | IDR 1,500,000 | same |
| Operating theatre use | IDR 1,000,000 | same |
For a foreigner paying out of pocket, the "non-regular" column doubles the consultation (IDR 80,000 instead of 40,000) and only shows the VIP and VVIP rooms: IDR 550,000 a day for VIP (about USD 34 at the sources' exchange rate, which tallies with the IDR 400,000 to 800,000 Bali Zero records for Sanglah) and IDR 1.5 million for VVIP. Classes III to I (IDR 210,000 to 350,000, roughly EUR 13 to 22) appear only in the BPJS column. Set against the USD 1,000 to 2,500 for a night in ICU at an international private hospital, public intensive care at IDR 550,000 costs roughly thirty to seventy times less — room only, once again.
Three caveats. These are room or procedure rates, without doctors' fees, drugs, consumables or tests: the final bill is always higher. The rate card rests on a 2024 tariff decision, still online as of 6 September 2026 and open to revision at any time. And the price buys you neither a shorter queue nor English-speaking staff — which is why France Diplomatie steers travellers towards private hospitals. Going public spares you the debt, not the discomfort.
The full rate card is published by RSUP Prof. Ngoerah, Denpasar's central public hospital, on the hospital's own site (in Indonesian).
Medical evacuation: when the bill enters another dimension
France Diplomatie spells it out: in the event of a serious health problem, an evacuation to Singapore or France "will have to be considered". The prices doing the rounds, from USD 20,000 to more than 150,000, stop contradicting each other the moment you separate two different products.
| Evacuation mode | Range | Source |
|---|---|---|
| Commercial flight with medical escort, Bali → Singapore | USD 20,000 – 40,000 | Insurance Indonesia, Jan. 2026 |
| Bali → Singapore, all options | USD 25,000 – 80,000 | Feather, June 2026 |
| Dedicated air ambulance, Bali → Singapore | USD 50,000 – 80,000 | April 2026 guide |
| Dedicated air ambulance, Bali → Singapore | USD 50,000 – 100,000 | Insurance Indonesia, Jan. 2026 |
| Air ambulance, Bali → Australia | USD 80,000 – 150,000 | April 2026 guide |
| From a remote island | up to USD 150,000 and more | Feather, June 2026 |
| Medical jet (French-language estimate) | around EUR 50,000 | vivreabali, June 2026 |
Bali Zero sums the whole thing up as "USD 20,000 to 100,000+": at the bottom, a seat on a scheduled flight with a medical escort; at the top, a dedicated, fully equipped air ambulance; the patient's condition decides. Feather, for its part, reckons the full sequence — a week in intensive care, then the evacuation — easily reaches USD 100,000. It is that kind of arithmetic that explains why France Diplomatie calls insurance "imperative".

How to read these numbers without getting burned
Four reflexes before you bank on any figure. Almost every source with numbers sells insurance: Insurance Indonesia is a broker, Feather an insurer, Europ Assistance an assistance company, and vivreabali's comparison tool earns affiliate commissions; the counterweights are the official RSUP Prof. Ngoerah rate card and the Bali Zero comparison. The exchange rate is dated: conversions from early and mid-2026, around IDR 16,000 to the dollar; redo the maths at today's rate. Two pages show no update date, Europ Assistance and Nomedic: figures cross-checked, freshness not guaranteed. A written quote before the procedure remains the best protection against a nasty surprise at discharge.
Preparing from abroad: what to sort out before you land
1. Cover, before you fly. France Diplomatie calls it "imperative". Line the caps up against the numbers: vivreabali puts the cover bundled with a French Gold bank card at EUR 155,000 maximum, while a week in ICU followed by an evacuation can reach USD 100,000 according to Feather. There is a margin, but it is not infinite: read the terms line by line.
2. A card that can take the deposit. Without a policy, or while you wait for the insurer's confirmation, the hospital will ask for a card to charge; a limit that cannot absorb USD 500 to 5,000 could leave you stuck at admission.
3. A hospital picked out before you need one. BIMC and Siloam are the most expensive and the only ones whose international accreditation is documented in our sources; Kasih Ibu and BaliMed cost two to three times less for a consultation; for Bali Royal, Surya Husadha and Prima Medika, none of our sources publishes a rate card. Note the address of the nearest one.
4. A line in the budget. A USD 50 to 75 consultation at BIMC is no drama; an unplanned USD 5,000 deposit is. Build healthcare into your Bali cost-of-living budget and tick the insurance line on our moving-to-Bali checklist before you buy the ticket.

Frequently asked questions
Can a hospital in Bali refuse to treat me without insurance?
In practice, yes, beyond emergency stabilisation. France Diplomatie states that Indonesian hospitals provide no care until they have a coverage agreement from an insurer or, failing that, a credit card number to charge. An April 2026 guide specifies that in the private sector, the financial guarantee is demanded once the patient has been stabilised.
What deposit does a private hospital ask for on admission?
The broker Insurance Indonesia puts the deposit demanded from an uninsured patient at USD 500 to 5,000 depending on the estimated cost of care, as a cash deposit or a bank-card guarantee (Bali hospitals guide, January 2026). It is the only source to put a figure on it, and it sells insurance; the range nonetheless squares with the rule reported by France Diplomatie.
How much does a night in hospital cost in Bali in 2026?
According to the Bali Zero comparison of February 2026, a night in a standard room costs IDR 3 to 5 million at BIMC (USD 188 to 313), IDR 1.5 to 3 million at Siloam, IDR 800,000 to 1.5 million at Kasih Ibu and IDR 400,000 to 800,000 at the public Sanglah hospital. Those amounts include neither doctors' fees, nor drugs, nor tests.
How much does an emergency-room visit cost in Bali?
Between IDR 100,000 and IDR 10 million depending on the hospital and the treatment. Bali Zero prices it at IDR 2 to 5 million at BIMC, IDR 500,000 to 2 million at Siloam, IDR 300,000 to 1 million at Kasih Ibu and IDR 100,000 to 500,000 at Sanglah; Feather goes up to IDR 10 million at BIMC.
Is Denpasar's public hospital really cheaper for a foreigner?
On paper, enormously so. The official RSUP Prof. Ngoerah rate card shows, in the "non-regular" (direct payment) column, a GP consultation at IDR 80,000, a VIP room at IDR 550,000 a day, intensive care at IDR 550,000 a day and operating-theatre use at IDR 1 million; class III to I rooms (IDR 210,000 to 350,000) appear only in the BPJS column. Those rates cover the room or the procedure alone, the rate card dates from 2024, and France Diplomatie advises preferring private hospitals. Cheaper, yes; comparable, no.
How much does a medical evacuation from Bali to Singapore cost?
USD 20,000 to 100,000 depending on the option. A commercial flight with a medical escort comes to USD 20,000 to 40,000 according to Insurance Indonesia; a dedicated air ambulance, USD 50,000 to 80,000 according to an April 2026 guide and USD 50,000 to 100,000 according to Insurance Indonesia. Feather gives USD 25,000 to 80,000, and up to USD 150,000 and more from a remote island. To Australia, count on USD 80,000 to 150,000.
What should I arrange before leaving so I am not stuck at admission?
Three things. Insurance or an assistance contract taken out before departure — France Diplomatie calls it imperative — with caps to compare against the USD 100,000 that a week in ICU followed by an evacuation can reach. A bank card whose limit can absorb a deposit of up to USD 5,000. And the address of the private hospital nearest your accommodation, among the six listed in Kuta, Nusa Dua, Denpasar and Tabanan.
